The U.S. Centers for Disease Control and Prevention has confirmed what you already guessed: This has been a remarkably mild flu season.
The influenza virus likes cold weather, so infections normally occur from October through March. But technically, the flu season doesn’t start until labs that test respiratory swabs from sick people find the virus in more than 10 percent of the samples.
This season, that threshold wasn’t reached until the week ended Feb. 11, making this the kindest flu spell in 29 years.
Pennsylvania, for example, had only 80 confirmed cases in all of January – barely more than one achy, feverish, nauseated citizen per county.
What’s going on?
No one really knows.
“With flu, everything is unpredictable,” said immunologist Scott Hensley, a flu expert at the Wistar Institute in Philadelphia. “I don’t think we’re out of the woods; it could just be a delayed season.”
Then again, maybe the flu has been as scarce as snow because snow has been scarce.
“Flu is more easily transmitted in colder temperatures. This has been a mild winter,” Hensley said.
Another theory is that the population has high levels of immunity to the influenza strains now circulating, which include the one that caused the 2009 “swine flu” pandemic. Because the strains have been so stable, people have had time to develop antibodies against them. Vaccination has also boosted immunity.
Although Hensley subscribes to this theory, he adds a caveat: “If that’s true . . . the virus will start mutating” to evade human defenses. “A novel strain might emerge in the next couple of months.”
While there’s no room for complacency, let us celebrate the signs, monitored by the CDC, that the flu has given the nation a respite:
One person per 100,000 has been hospitalized with the flu since October. That’s a 95 percent drop from last season’s rate of 22 people per 100,000.
Only 1 percent to 2 percent of visits to doctors since October have been for flulike illness. The usual rate is 3 percent to 8 percent.
This flu season, there have been three flu-related deaths among children, compared with 122 pediatric deaths last season – and 348 during the 2009 pandemic.

Where The Boys Are: Merck, Gardasil And HPV

In the midst of ongoing controversy over HPV vaccines, the Advisory Committee on Immunization Practices, which advises the US Centers for Disease Control and Prevention, voted unanimously to recommend routine use of Merck’s Gardasil in boys as young as 11 years old. The vaccine fights human papillomavirus, which causes genital warts and oral, penile and anal cancers in men and cervical cancers in women.

Two years ago, Gardasil was first approved for in males between ages 9 and 26 to combat two HPV strains that can cause genital warts, so the new recommendation greatly expands the population, although the ACIP previously suggested vaccination was suitable for boys. As for females, the Merck vaccine was originally approved in 2006 to protect against four HPV strains that can lead to cervical and other cancers in girls and women in the same age range.

The move comes just weeks after Gardasil took center stage in the Republican presidential primary campaign. Texas Governor and GOP candidate Rick Perry backpedaled on a 2007 decision to mandate Gardasil for school-age girls, and his ties to Merck at the time became campaign fodder. Later, rival candidate Michele Bachmann claimed Gardasil causes brain damage (see here and here).

The ruckus reflected concerns over side effects, long-term benefits and views of social conservatives, who fear vaccination will give teenagers a green light to premarital sex. This last issue was debated in California over the past several months, where a bill was introduced that removed parental consent for vaccinating children 12 and older against sexually transmitted diseases. Earlier this month, California Governor Jerry Brown signed it into law, but not before the debate raised charges that Merck engaged in the sort of surreptitious lobbying that was conducted when Gardasil was approved in 2006 (back story).

This is the second bit of good news that Merck has received this week about Gardasil. Early-stage research found evidence of a link between HPV infection and heart disease in women who have no other risk factors. The study appeared in the Journal of the American College of Cardiology (see here).

Last year, Gardasil sales slowed considerably, from $1.4 billion in 2008 to $988 million, although for the first half of this year, revenue climbed about 10 percent, to $490 million, from a year earlier. Besides controversy, cost has also been a factor – Gardasil costs about $360 for a three-course dose. The drugmaker will welcome the increased sales that come with the ACIP recommendation, given that 13,000 jobs are in the process of being eliminated. In fact, Merck employees in numerous locations this week are being notified of pending layoffs.

Reglamento Sanitario Internacional (2005)

Con el apoyo de la OMS, los 194 Estados Partes en el Reglamento Sanitario Internacional aplican esas normas mundiales con el fin de reforzar la seguridad sanitaria nacional, regional y mundial.

Los hitos principales para los países serán la evaluación de sus capacidades de vigilancia y respuesta antes del fin de junio de 2009 y la elaboración y aplicación de planes de acción para velar por que esas capacidades básicas estén en funcionamiento antes de que finalice 2012. http://www.who.int/entity/ihr/IHR_2005_es.pdf

Entrando en la página de la OMS, aparece un informe de los avances en el reglamento sanitario internacional. Esperaba encontrar aspectos generales sobre el abordaje de distintos problemas, pero para mi sorpresa, se trata de un documento actualizado el 9 de Abril de 2009  (es decir casi 10 dias antes que aparecieran los primeros casos de esta pandemia de gripe A N1H1). Y que se refiere justamente a como actuar frente a la gripe A (N1H1), insisto, todo esto 10 dias antes de que en México se declararan las alertas. Leo el punto 15 y habla de la preocupación existente debido a la resistencia que el virus de la gripe estacional N1H1, tiene frente al Oseltamivir.
No me queda muy claro este asunto. Un reglamento sanitario internacional supongo que debe ser algo generico para que los gobiernos encaren distintos problemas de salud. Pero en este documento sólo se refiere a la gripe, algo que para la fecha de edición del documento no parecia constituir un problema (aunque si la OMS venia insistiendo sobre la gripe Aviar desde muchos años antes).
Luego de leer el punto 15 mi asombro es mayor. Habla especificamente del virus N1H1, cuando hasta esos dias la gripe estacional se daba mayoritariamente por el virus N3H2. Dice que hay preocupación por la resistencia del Oseltamivir ante este virus. Y 2 semanas mas tarde, el mundo se conmoveria por la aparicion de un nuevo virus (que no es del todo tan nuevo), en plena ciudad de México, quien cerraria todas sus puertas, aisló a su población, y medicó a todo el que pudo. Parece que la OMS no sólo es un organismo serio, sino también omnisciente y capaz de preveer el futuro.  Lo sucedido en México, paso a ser un ejemplo de decisión para la propia OMS, no importaron los 30 mil millones de dolares que perdió, debido a la gripe, sin contar los muertos, obviamente. Pero en el caso del impacto económico, esto fue subsanado rapidamente por un crédito del Banco Mundial, por la misma cifra calculada. Un epidemiologo Mexicano nos contaba hace meses en Argentina, lo que pasaba en plena epidemia, donde la OPS lo mando a ayudar, y también a aprender sobre la epidemia, y lo podemos ver aun en video.
Por entonces Argentina era el centro del mundo, como bien dice el periodista, historia conocida de la que ya hablamos varias veces. El propio Google decidio dejar de actualizar su google map y lo dejó en manos en Flutracker, un mapa que recibe información desde distintos lugares del mundo, y que convoca a cualquiera a informar sobre casos probables, posibles, confirmados o lo que sea. En Argentina muestra 511 muertes contra las 600 que declaro el Ministro de Salud, luego que una periodista de Clarin publicara dos muertos en la provincia de Buenos Aires, y el resto se sumara luego a esta noticia, que llevo al gobierno a contar lo que venimos publicando desde hace meses. La gripe en Argentina no desaparecio. Hace tan solo dos dias publicamos noticias de todo el mundo, en donde hasta un gato habia muerto en Iowa. En ese mismo articulo leiamos que la cantidad de casos en China se habia triplicado. El problema era saber desde que numero se habia triplicado. Desconozco las fuentes, pero un blog reporta ayer lo siguiente: “China confirmó hasta la semana pasada 55.000 casos de influenza A, entre los cuales hubo 16 muertes; un 75 por ciento de los afectados se ha recuperado y 176 personas se encuentran en estado grave, según datos del Ministerio de Salud.”.
Información que no queda clara cual es la fuente donde obtuvo ese dato, pero asumamos que son cifras ciertas. Si uno saca las cuentas, y a esta hora pocas ganas tengo, la letalidad parece más que inferior al resto de los datos a nivel mundial que estiman la mortalidad en un 0,5%..
Hoy discutia en una lista este tema, y alguien me refutó que no puede ser que todos mientan. Y creo que tiene razon. Tanto gobiernos, como CDC, como OMS, no pueden estar conspirando a la vez. Aunque no es menos cierto, que las comunicaciones resultan esquizofrenicas, llaman a la calma, y a la vez publican medidas para evitar mayores calamidades. Si algo tiene Argentina para mostrar, es justamente que todo se hizo al reves. Y superamos el pico de la epidemia. Los gobernantes se adjudican el triunfo. Los mercaderes de la salud también. Aunque sus medicamentos no hayan servido, las consultas por gripe ya estaban bajando cuando se tomaron las medidas de medicar, cambiar los criterios de gripe por ETI (enfermedad tipo influenza), y suspender las clases, y todo tipo de reuniones. 
Esta epidemia, a la que siempre he llamado de pánico, creo que es más una muestra de la irracionalidad que subyace naturalmente en cada persona, en cada familia, en cada sociedad. Sin duda muchos gobiernos se benefician ocultando cosas, asi como las farmaceuticas vendiendo. Quizas en la pantalla de quien esto lea en este momento, la publicidad contextual de Google les muestre las mil y una formas de evitar la gripe. La primera pandemia en la era de la información, da por resultado quee nos hemos intoxicado con ella. Y como alguien dijo al principio de esta epidemia: “la epidemia terminará cuando los medios dejen de hablar de ella”. Pero los medios se nutren de información oficial en muchos casos. Y en otros, como en esta ciudad, un pasquin, que casualmente esta comprometido con una clinica (son los mismos dueños), se encargaba de informar supuestos casos de gripe que  nunca fueron confirmados. Pero la memoria es corta.
El viejo mapa de google sobre la gripe, consistia en predecir la dispersion de la epidemia en funcion de las propias busquedas que  la gente hace. Pues bien, en Argentina hay gripe, y las búsquedas indican que los niveles son minimos. La gente se siente segura porque lo peor ya pasó. Y asi como el Norte parece no haber aprendido de la experiencia del hemisferio sur. El panico se ha expandido aún frente al deseo de muchos otros bloguers, en particular de españa, que convocaron bajo el lema Gripe y Calma.
Es posible que China mienta, siempre lo ha hecho cuando de politicas (en este caso sanitarias) se trata. Los bajos niveles de gripe que muestra Africa se correlacionan más con la falta de centros de vigilancia epidemiologica, y de acceso a internet, que con la propia pandemia. Las embarazadas y los obesos aparecen como nuevos grupos de riesgo, pero nadie puede contestar cuantas embarazadas y obesos morian de gripe cada año. Del mismo modo, en uno de los primeros reportes de Argentina, se ponia como grupo de riesgo a los pobres. Sin embargo, excepto Australia, no se encuentra ningun reporte que diga que los pobres esten muriendo más. Largas colas de personas en edificios publicos se  juntan en Argentina para acceder al subsidio universal para niños pobres, cual fue la letalidad en estos grupos? Un subsidio que de por si es humillante, aunque sin dudas algo hay que hacer. Que paso con ellos durante el pico de la epidemia? Que esta pasando hoy, cuando la pobreza es del 32% y el virus N1H1, ha desplazado por completo al H2N3, y aún a otros virus. Excepto en niños menores de 5 años donde el 23% es gripe A.
Esta semana murieron 5 personas. La única diferencia con la gripe estacional es que este virus seguira presente aun en verano. Ayer 5 personas se internaron en Rosario probablemente por Dengue. Como ha dicho la Dra. Chan: “la gripe se cura sola”, a la vez que proclamaba los beneficios del medicamento, y hoy los de la vacuna. Medicamentos que no curan, pero que disminuyen en medio dia el tiempo de enfermedad.
Pero no hay medicamentos para el dengue, y es conocido que en general la primoinfeccion provoca sintomas gripales (lo que habra de confundir a mas de uno si es gripe o dengue), pero la segunda vez que alguien es picado por un mosquito, la forma hemorragica es la forma mas frecuente y con mayor letalidad. Esperemos esta vez, no repetir la historia de los ultimos dias de abril, cuando el dengue se fue, no por las medidas tomadas como dijo la entonces Ministra, sino por el frio.
